An error is a mistake, fault, failure or shortfall that creates an unexpected, unstable, unacceptable or generally negative outcome. Errors are unintended and result from complex factors that can have human, computer, process, procedural, system and natural root causes. The following are common types of error.
Human ErrorHuman errors are mistakes made by a person. This occurs when a person doesn't do what could be reasonably expected in a situation. Human errors have many causes such as distraction, low diligence or ability shortfalls. All humans regularly make mistakes but there are important situations where people are entrusted not to make a mistake. For example, a pilot landing a plane can be reasonably expected to not make a major mistake due to their professionalism and training.Latent Human ErrorsLatent human errors are features of products, systems, environments, processes and institutions that make human errors likely. The root cause of such errors is not human error but the error-prone thing. For example, a device that allows users to put batteries in backwards but if they do, the battery gets stuck and the device is ruined. It is not reasonable to blame this on the user.Design ErrorsDesigns that produce invalid outcomes, inefficiencies, failures or instability.
