A decision rationale is a document that describes the reasons for a decision. It is a common condition of many organizations that nobody knows why past decisions were made. In other words, the knowledge generated as part of a decision making process may not be documented with only the decision itself surviving. This loss of knowledge may cause an organization to repeat painful and expensive learning processes.
