A self assessment is an evaluation of one's own performance. This is often conducted as part of a performance management process whereby employees first evaluate themselves and are then evaluated by management often with feedback from stakeholders. A self assessment is typically designed to be positive. They may make use of direct language, action verbs and specific measurements of outcomes in order to pitch for a favorable performance review. The following are illustrative examples of self assessments.

Organization

Organized annual team building event including the design of activities. Feedback indicated that 82% of participants felt the event will improve team morale and productivity.

Planning

Developed a business plan for improvements to the sales system with an estimated return on investment of 440%.

Time Management

Notes

A self assessment typically addresses each of your performance objectives in a paragraph format possibly with a point form list.
Most of the examples above include a measurement as this is ideal. There are also several examples that include no measurement as illustrations of how this can be approached. In many cases, using any numbers at all can be perceived as a measurement of results. For example, consider the following two accomplishments:
Successfully delivered the ABC project.
Developed and launched 23 product features for the ABC project with 0 defects in production.
They both say the same thing but one has a few numbers that can be perceived positively.
